Douglas County Fire District No. 2 shall be held harmless from events beyond its control. Any non -district affiliated groups or persons wishing to use the meeting room must fill out this form and agree to the following conditions: 1. Parking for those using the meeting room shall be at the east end of the parking lot off N.E. 4th St. 2. The user will be held responsible for all damages which occur as a result of their use of the facilities. 3. There will be no running or horseplay in or around the fire station. 4. The use of the meeting room will also include the use of the public restrooms located adjacent to the meeting room. Access to other portions of the building shall be by authorized invitation only. 5. The meeting room, restrooms, and connecting corridor are "non-smoking" areas. Smokeless tobacco use is also prohibited. 6. Alcoholic beverages will not be allowed on the premises. 7. Animals inside the facility will be limited to service dogs. The named group or persons shall provide Douglas County Fire District No. 2 with a Certificate of General Liability Insurance and/or name Douglas County Fire District No. 2 as an additional named insured for the times the facilities are being used by the Group. The Waiver at the end of this form may be used for this purpose. 9. Following the use of the facility, tables and chairs shall be returned to a classroom setting, garbage removed and the room left clean. The following housekeeping items are requested: 4/2018 - 1 - *Spray and wipe down all table surfaces then return chairs to table tops. *Wipe down meeting room sink and counter top. *If garbage bag is full, please remove and set next to rear door; replace with new bag. *NOTE: Garbage liners, disinfectant spray, and rags are supplied and located on the sink counter. Please leave dirty rags on the floor at rear door. In the rear of the room, you'll find a sink, paper towels, and a garbage receptacle. The restrooms are located down the hall, just outside of the meeting room. Extra chairs are stored in the rear of the meeting room on a mobile cart. Wi-Fi access is permitted; login and password are posted at the front of the room. During use of the facility, the access to the remainder of the firehouse is typically locked. This is simply a security measure for the on -shift crew while out of the building for emergency calls or other duties. If there are any special needs, please let the Shift Officer in charge know upon arrival for assistance.
