Change Description
Originated as RFI #5: Time Capsule Excavation, RFI #6: Existing Junction Box Grade, RFI #7:
Pothole Tree Location, and RFP #5: Existing Sign Base Removal.
Intent of this change order was to combine separate unforeseen removal items into one change
order.
Contract Plan Sheets: No Change
Construction Requirements/Special Provisions: No Change
Change in Contract Costs: The cost for this Change Order is the cost of KCRI work for unforseen
removal items including concrete removal and haul, additional excavation, potholing in tree locations,
removing sign base, lowering existing J-box, and trimming concrete around existing light base.
Contractor provided Force Account tracking information for work.
Concrete removal/haul = $4,497.56
Time capsule additional excavation = $238.49
Tree potholing = $418.82
Sign base removal = $345.27
Lower existing j-box = $239.96
Light base trimming = $102.10
Total Cost = $5,842.21

Brief Description of Problem / Reason for Entitlement:
Concrete removal/haul: Unforeseen concrete was found in various locations on site and had to be removed and hauled off
site.
Time capsule additional excavation: During excavation for the time capsule, a 18" diameter concrete pipe was found 2-2.5'
below grade. Additional excavation was required to move the capsule over approximately 2'.
Tree potholing: Due to concrete found on site, tree locations along 9th Street were potholed to identify potential planting
issues. Concrete was found in 1 of 3 locations and its removal was tracked under FA information for "Concrete
removal/haul".
Sign base removal: Former Texaco sign base was found to be above finish grade and had to be cut down.
Lower existing J-box: Existing junction box was found to be 0.2' above finish grade and was lowered to eliminate tripping
hazard in sidewalk.
Light base trimming: The existing light base was surrounded in concrete and additional work had to be done to create clean
edges for concrete sidewalk to be poured around.
Justification of Cost:
KCRI work was tracked via force accounts, in the exception of the sign base removal, which was
an agreed upon lump sum price.
Concrete removal/haul = $4,497.56 (FA tracking)
Time capsule additional excavation = $238.49 (FA tracking)
Tree potholing = $418.82 (FA tracking)
Sign base removal = $345.27 (LS price)
Lower existing j-box = $239.96 (FA tracking)
Light base trimming = $102.10 (FA tracking)
Total = $5,842.21
